This is a checklist of the document groups required for Proof of Residence, should you be requested by SLL to provide if needed for verification (i.e. for All-Stars Eligibility). Documentation is optional during the online registration process, however you will be required to affirm the address provided in Sports Connect is your current residence. Please use the League Boundary Finder to confirm your residence is within the SLL boundary prior to registering online.

This form is used if you choose "school" as your method of eligibility during online registration and you are requested by SLL to provide when needed for verification (i.e. for All-Stars Eligibility). Documentation is optional during the online registration process, however you will be required to affirm the school's address is within the SLL boundary. Please use the League Boundary Finder to confirm the school's address is within boundaries prior to registering online.

It is IMPORTANT to know that this form MUST be completed and signed by a Parent or Guardian and given to the coach BEFORE participation in any organized practice or game.

This form is negated by the online volunteer sign-up process, but is here in the case a need arises for a paper application. If you have not signed up during registering a player for the current season, you can log into your Sports Connect account, click "Volunteer" off to the left and follow the prompts.
